Dr. Eric Anderson of Chicago’s Impressions Face + Body told the Daily Mail that many patients report a strange musty or sour odor after their BBL. According to South Shore Plastic Surgery, this smell often becomes noticeable when patients sweat or sit for extended periods.

However, it’s not just harmless odor. Anderson points out that in some cases, the smell could be linked to fat necrosis — a complication where transferred fat tissue dies. “When too much fat is injected and the blood supply isn’t enough, the fat can die, leading to a rancid smell,” he explained. This condition can result in infections requiring antibiotics, hospital stays, and in severe cases, sepsis.

Hygiene Challenges After Surgery

Another factor contributing to the smell is post-surgical hygiene. Patients sometimes find it difficult to clean their buttocks properly after a BBL. If residual stool or urine remains on the skin, bacteria can grow and create a noticeable odor. Experts at South Shore Plastic Surgery advise using fragrance-free wipes or a bidet for thorough cleaning to avoid these issues.

Despite these risks, many still choose to undergo the surgery — even more than once. TikTok user Scarlet Black shared her experience of needing two BBLs because the first procedure didn’t last. “A lot of the fat re-injected into your buttocks dies — sometimes 50% or more,” she said. Black admitted she spent $36,000 on plastic surgeries to achieve her desired look and didn’t mind going under the knife twice.
